Now a new series of shorter and easier applications have been released by CMS. Stinging from criticism that the new Health Care applications were too long and too complicated, the Obama administration went back to the drawing board. The new individual application has been reduced from the 21-page original draft to just 3-pages long and the family application was reduced by 2/3rds to just 7-pages long.

Depending on your circumstances, you will need to choose one of three applications to enroll for health care under the Affordable Care Act:  the individual short form, the family application  or the application for individuals without financial assistance.
